Labs
Admission: CBC, CMP, LDH, uric acid, Phos, PT, PTT, INR, fibrinogen
Tumor Lysis Labs: CMP, Phos, uric acid, LDH
Daily: CBC, CMP, +/- TLS
Transfusions
Obtain blood product consent (consult?)
Leukoreduced, irradiated for all heme patients; future BM transplant candidates with negative CMV IgG should also receive CMV safe blood products
Normal pRBC: Hgb < 7 or symptomatic anemia
Normal platelets: <10, <20 if febrile, <50 if planned procedures/bleeding
IV Access
Re-evaluate lines daily and need for central lines
Discuss with fellow if PICC is required for chemotherapy
For patients requiring PICC, consent prior to placing order
Chemo Regimens
Ask fellow for regimen and potential toxicities
Know cycle and day of chemo administered, anticipated discharge
Antimicrobial Prophylaxis
Neutropenic ALL: acyclovir 400 mg PO BID, fluconazole 400 mg QD, Bactrim DS qMWF, levofloxacin 500 mg QD
Neutropenic AML: acyclovir 400 mg PO BID, voriconazole 400 mg QD, levofloxacin 500 mg QD
