Deep Vein Thrombosis
Therapy
- Initial anticoagulation: immediately for up to 10 days
- Long-term anticoagulation: minimum of 3 months, sometimes 6-12 months; occasional indefinitely
- Options
- Heparin
- LWMH
- SubQ
- DoAC
- Apixaban
- Recurrence: 3 per 100 person-years
- Major bleed: 3 per 100 person-years
- peak-to-trough: 3
- Rivaroxaban
- Recurrence: 7 per 100 person-years
- Major bleed: 7 per 100 person-years
- peak-to-trough: 10
Other Options
- Thrombolysis: not routinely performed
- Indications (thrombolysis/thrombectomy)
- phlegmasia cerulea dolens
- massive iliofemoral DVT
- failure of therapeutic anticoagulation
- candidates:
- symptoms < 14 days
- good functional status
- low bleeding risk
