Deep Vein Thrombosis

Therapy

  • Initial anticoagulation: immediately for up to 10 days
  • Long-term anticoagulation: minimum of 3 months, sometimes 6-12 months; occasional indefinitely
  • Options
    • Heparin
    • LWMH
    • SubQ
    • DoAC
      • Apixaban
        • Recurrence: 3 per 100 person-years
        • Major bleed: 3 per 100 person-years
        • peak-to-trough: 3
      • Rivaroxaban
        • Recurrence: 7 per 100 person-years
        • Major bleed: 7 per 100 person-years
        • peak-to-trough: 10

Other Options

  • Thrombolysis: not routinely performed
  • Indications (thrombolysis/thrombectomy)
    • phlegmasia cerulea dolens
    • massive iliofemoral DVT
    • failure of therapeutic anticoagulation
  • candidates:
    • symptoms < 14 days
    • good functional status
    • low bleeding risk
