Transfusion Reactions

Reactions Description
Benign febrile non-hemolytic transfusion reaction Fever immediately
Acute hemolytic transfusion reaction Fever immediately, followed by
Tachycardia, shock, flank pain, hemoglobinuria (muddy brown urine without RBCs)
Delayed Hemolytic Transfusion Reaction
Delayed Serologic Transfusion Reaction (no hemolysis)
Days to weeks
Jaundice, pallor, fatigue
IgE-mediated allergic reaction Hives, urticaria, anaphylaxis
Transfusion-transmitted bloodstream infection

Management

  • Concern for acute hemolytic transfusion reaction
    1. Stop transfusion
    2. Take care of patient (meds, fluids, oxygen)
    3. Call blood bank
      1. Peripheral smear
      2. Direct/indirect Coombs
      3. Other hemolysis labs

Other

  • All pRBCs are leukoreduced at LAC+USC
  • Preparing for Blood
    • Blood type: ABO/Rh status, <15 minutes (for lab)
    • Screen: 15-30 minutes (for lab)
    • Cross: 30-60 minutes (for lab), highly variable
